Industrial Accident Investigation Procedures

In this article, we will learn about how to investigate an industrial accident. Within this article, we use the duration of the accident, in order to have an event that injury, property damage and to describe causes of death.

It is extremely important in an industrial accident or normal workplace to find the cause of the accident. All accidents that occur in both major and minor accidents, the plant site to be informed as soon as possible.

Determine the root cause of an accident, workplace accident, is one of the most important aspects of the investigation.

With the underlying cause may be an underlying cause of the accident.Let's take an example. Some people may agree that crash on a wet floor (slips) was due to the wet floor. It really does not cause the immediate root cause.

What we have to determine the root cause or underlying cause is not immediately. In the example above, we have to determine why the water was on the floor. Water leaking valves, pipes, or you come from? Or water is spilled? How long did the water on the floor before the accident? This information is more important and useful.

By knowing the root cause, we get many benefits for our long-term safety, as we have already known that in the future will be to prevent such accidents.

Industrial accident investigation process

Well, now we will talk about industrial accident investigation procedures. There are five main steps of procedures, the reporting, information gathering, information analysis, risk control measures established to identify and plan of action and its implementation.

Reporting

Reporting is an important first step in a workplace accident investigation. It is possible and should be as complete as the extension.

The effect of media reporting, including reporting and time.

Collecting information

Workers who were involved or injured in the accident and supervisors are the main source of information. Includes gathering interviews.

In addition to interviews, accident scene photos and pictures can also be used to collect information. During this stage, we are always what they should remember the five W's and one H., who, where, when, why and how.

Here are some examples of questions in order to gather information about the accident:

• What happened?

• When the accident happened?

• Who was involved in the accident?

• Who were the supervisors?

• where the accident happened?

• Workers were doing before the accident and the accident happened?

• Machine tools, equipment, the incident was being used?

• Workers have followed standard operating procedure?

• workers received training related to the operation?

• Were there any injured workers?

• how do injured workers?

• workers wear appropriate personal protective equipment, what?

Injured workers at the site of first aid measures?

• Were there any other person who saw the accident?

• accident (the sequence of events), how?

Anyone in the sequence of events, which investigates industrial accidents for the most important question. Immediate cause of most cases of workplace accident rather than something that went wrong in the past is due.

Information Analysis

Collect information related to the accident, the information is analyzed by the investigators. In this phase, the investigators analyzed the sequence of events clearly beginning to understand why the accident happened. Finally, investigators have determined the underlying causes of the accident.

A common way to analyze the sequence of events is usually the tree diagram used by several accident investigators. Tree diagram is placed on top of accident and contributing factors are the branches. The root cause of the accident lies at the root of the tree. So that's why it is extremely important to know the root cause of the accident to prevent recurrence.

Identify risk control measures

Once you have identified the root causes of the accident, we control measures or preventive action to cut off the roots and prevent recurrence of the accident should provide.

The best control measure is to eliminate risk at all. If eliminating the risk is not possible, try to reduce the risk. If the risk is still present, we have to reduce risk exposure to workers. Mainly, the risk or hazard control hierarchy to follow when determining the best control measures.

Action plan and the establishment of implementation

Once a plan is in place and control measures scheduled for implementation are determined. This is the final stage of industrial accident investigation process.
